D. Hugo and Associates Inc. acting as third party administrator (TPA) with Littauer Occupational Alliance (LOA) located at Nathan Littauer Hospital will provide the following screenings under Dr. Evars, Medical Review Officer, and Brenda Ward, Occupational Health Co-ordinator.

 

D. Hugo and Associates Inc. acting as third party administrator (TPA) with Littauer Occupational Alliance (LOA) located at Nathan Littauer Hospital will provide the following screenings under Dr. Evars, Medical Review Officer, and Brenda Ward, Occupational Health Co-ordinator.

Drug and Alcohol Screenings:

a. Pre-Employment driver screenings
b. Random pool screenings
c. Reasonable suspicion screenings
d. Post accident screenings

Littauer Occupational Alliance will perform driver screenings at the following rates:

Drug screening .....................$45 dollars per driver
Alcohol screening ............... .$25 dollars per driver
Confirmation positive test ... $25 dollars per driver.
Littauer Occupational Alliance performs
DOT driver physicals ............. $48 dollars per driver.

**Note: Federal DOT regulations require driver physicals be performed every two years.

All service provided by Littauer Occupational Alliance, screenings and physicals, are the responsibility of the company and will be paid directly to Littauer Occupational Alliance, and can not billed to D. Hugo and Associates Inc.
